Custom application and signup forms

📝 Application Forms System

Create professional application forms for staff positions, events, or any other purpose. Features automatic role assignment, review workflows, customizable embeds, and comprehensive analytics. Every new form appears instantly in the dashboard without needing a manual refresh.

How to Set Up Application Forms (Step-by-Step)

Follow these steps. Each step tells you exactly where to click in the dashboard.

Open the Dashboard

Go to modora.gg/dashboard and select your server. If Application Forms is disabled, open Modules, enable it, then click the Application Forms card.

Open Application Forms

You're now on the Application Forms page. Use the sidebar or module overview to open it.

Create a New Form

Click 'Create New Form' or '+ New Form'. A step-by-step modal will guide you through the process.

Fill in Basic Information

Enter name, description, emoji, and color for your form. Helps users identify your form easily.

Configure Settings

Set submission limits, time limits, cooldowns, and link to ticket panels if needed. Configure linked forms here.

Add Questions (Optional)

Add questions: short text, long text, select, multi-select, or boolean. Questions are optional - you can create a form without any.

Customize Panel Embed

Customize how the application panel looks in Discord. Set title, description, color, footer, images, and more.

Configure Role Management

Set required roles, blocked roles, reviewer roles, and role actions (add/remove/submit on approval/denial).

Review and Create

Review all settings, then click 'Create Form' to finish.

Deploy to Discord

Click 'Deploy Panel' on your form, select a channel. The panel appears in Discord with a button for users to click.

✅ That's it!

Members can now click the button in that channel to start an application. They'll be guided through the questions, and staff can review and approve/deny applications.

What Can Application Forms Do?

Unlimited Questions

Add as many questions as you need.

Short text, long text, select dropdown, multi-select, and boolean (yes/no) questions

Approve/Deny applications with custom reasons. Automatic role assignment based on approval status.

Role Management

Set roles required to create applications, roles blocked from creating, reviewer roles, and automatic role actions (add/remove/submit roles)

Customize DM messages for submission, approval, and denial. Configure logging channels for staff actions.

Custom Embeds

Fully customize panel embeds, DM notification embeds, and log embeds with titles, descriptions, colors, images, and more

Linked Forms

Link multiple application forms together, similar to Discord's application system. Users can choose between linked forms.

Time Limits & Cooldowns

Set submission time limits and cooldowns between applications. Automatically deny applications that exceed time limits.

Analytics Dashboard

View comprehensive analytics including growth charts, approval rates, and submission statistics

History Tracking

View application history for users and staff. Track all submissions and reviews in one place.

Ticket Integration

Link application forms to ticket panels. Staff can create tickets directly from application review embeds.

Access Control

Control who can create applications with role requirements and blocks. Set maximum pending applications per user.

Global Settings

Global settings apply to all application forms in your server. Configure these in **Dashboard → Application Forms → Settings** tab.

Available Settings:

  • **Cooldown Time:** Delay between executing application commands (in seconds)
  • **Deny on User Leave:** Automatically deny pending applications when a user leaves the server
  • **Default Accept/Deny Reasons:** Pre-filled reasons when approving or denying applications
  • **/apply Command:** Enable or disable the /apply command globally. When enabled, you can select which forms are available via the command.
  • **Custom Embed Configurations:** Customize embeds for Application Submitted, Application Approved, and Application Denied notifications

Role Actions Explained

Role actions allow you to automatically manage user roles based on application status. Configure these in the **Roles** step when creating or editing a form.

✅ Submit Role

Roles assigned when a user submits an application (while it's pending). These roles are automatically removed when the application is approved or denied.

**Use case:** Give applicants a "Pending Application" role so they can access a waiting area channel.

➕ Add Role

Roles assigned when an application is **approved**. These roles remain assigned to the user.

**Use case:** Give approved applicants their new role (e.g., "Staff", "Member", "Event Organizer").

➖ Remove Role

Roles removed when an application is **approved**. Note: Remove roles are only processed on approval, not on denial.

**Use case:** Remove an "Applicant" role when someone is approved for a staff position.

Application Commands

Here are all the commands related to application forms:

👤 Commands for Users

Anyone can use these commands to view their own application history

/apply [form-name]

Start an application using the specified form. Only works if the /apply command is enabled in Global Settings and the form is available.

/apply Staff Application

/userhistory applications [user] [range]

View application history for yourself or another user. Range: 1d, 7d, 2w, 30d, 3m, 1y, or all.

👔 Commands for Staff

Staff members can use these commands to view application handling history

/staffhistory applications [user] [range]

View how many applications a staff member has reviewed. Range: today, 1d, 7d, 2w, 30d, 3m, 1y, or all.

Reviewing Applications

When an application is submitted, it appears in your configured logging channel (or review channel) with Approve, Deny, View Details, Open Ticket, and User History buttons.

Review Process:

Application is submitted and appears in your logging/review channel

Click **"View Details"** to see all answers and applicant information

Click **"Approve"** or **"Deny"** button

Add a review note (optional - default reasons will be used if not provided)

Confirm the action. Roles will be assigned/removed automatically based on your configuration

**Note:** Only users with the configured Reviewer Role can approve/deny applications. If no reviewer role is set, any staff member can review.

Panel Types

You can choose between two panel types when deploying your application form:

🔘 Button Panel

A single button that users click to start the application. You can customize the button label, style (Primary, Secondary, Success, Danger), and emoji.

📋 Dropdown Panel

A dropdown menu that appears when users click the button. Useful when you have linked forms - users can select which form they want to apply for from the dropdown.

Common Questions

Tips & Best Practices

  • **Start Simple:** Create a basic form first, then add more questions and features as needed.
  • **Use Clear Questions:** Make sure your questions are easy to understand. Add descriptions to help applicants know what you're looking for.
  • **Set Reasonable Limits:** Don't set submission time limits too short, or applicants might not have enough time to complete the form.
  • **Customize Your Embeds:** Make your panel embeds attractive and informative.
  • **Use Default Reasons:** Set default accept/deny reasons in Global Settings to save time when reviewing applications.
  • **Review Regularly:** Check your Analytics tab to see application trends and approval rates. This helps you improve your forms.
  • **Test Before Deploying:** Create a test form and submit it yourself to make sure everything works as expected before making it public.
